Health Partners is dedicated to creating a healthier community by educating
people on key healthcare issues and on the role every one of us can play to
build a more efficient, cost-effective healthcare system.
Our Beliefs on Prevention
and Disease Management
- Emphasize and make available to everyone primary and preventative care as
well as disease management.
Because keeping people healthy is part of our Mission, Community Health Partners conducts health
programs throughout the communities we serve. Screenings can help catch
cancers and heart disease early, making them easier to treat successfully.
- The Women's Health Center and Parish Nursing Program reach out to train
Latino and African American women on the importance of annual mammograms.
- Community Health Partners was the first to take breast imaging to a higher
level with the Breast MRI program and Computer Aided Detection program, two
of the latest tools available for early detection of breast cancer.
- The Resource Mothers' Program recruits and trains local women, many who
were single mothers dependent on public assistance, to serve as mentors, advocates
and friends to guide the program's clients to healthier lifestyles.
Because we believe better management of chronic diseases could improve people's
quality of life and reduce healthcare costs, Community Health Partners works
to educate patients with chronic diseases like Heart Failure, Chronic Obstructive
Pulmonary Disease (COPD) and Diabetes.
- Community Health Partners Cardiopulmonary Rehabilitation Programs assist
patients with acute or chronic cardiac and pulmonary disease to maintain independence
and increase endurance for everyday activities.
- Community Health Partners Parish Nurse program works with churches to provide
diabetes education and management for at risk patients within minority populations.
- Community Health Partners Resource Mothers' Program educates and mentors
families at risk by providing tools and resources necessary to make good decisions
and lead healthy lives.

- Know your numbers. Have blood pressure, blood sugar and cholesterol levels
checked.
- An ounce of prevention is worth more than a pound of cure. Preventive tests
such as mammograms, prostate exams and colonoscopies save lives.
- Stay well. If you're healthy now, do all you can to stay that way. Have
annual physical exams, eat right, get proper exercise and don't smoke!
