Patient-Centered Care

Community Health Partners is dedicated to creating a healthier community by educating people on key healthcare issues and on the role every one of us can play to build a more efficient, cost-effective healthcare system.

Our Beliefs on Patient-Centered Care

  • Put patients and families at the center of the care process - respecting the uniqueness of all individuals by considering their physical, psychological and spiritual needs.

We believe patients' needs should always come first. As a not-for-profit organization, we reinvest what we earn back into the care and community services we provide. We also work to deliver our care in a manner that reflects our sensitivity to a patient's medical and personal needs.

The way Community Cancer Center approaches fighting cancer is a great example. More than investing in state-of-the-art imaging and treatment technology (IMRT/IGRT), our Cancer Centers are founded on treating the whole person - mind, body and spirit.

Believe it? Live it. Lend a hand.

Look for reports on hospital performance, particularly on patient satisfaction. Talk to co-workers, friends and family about their hospital experiences. Discover if they treat patients with respect and dignity. Learn if they keep patients and loved ones informed. Do patients feel like their voice is heard or their opinion matters?
