The Community Health Partners Regional Foundation
Local Focus
The Community Health Partners Foundation is a non-profit 501 (c)(3) charitable organization established to provide direct philanthropic support to Community Health Partners. The Foundation is governed by a twenty-member board of trustees comprised of local business and community leaders. Foundation endowments and program support funds have been raised through the thoughtful generosity of many donors and are invested and managed by local institutions.
A Strong Heritage
Community Health Partners Foundation grants are made to programs and services of Community Health Partners which foster the achievement of excellence in community-based health care delivery and advance our mission and values. Foundation grant support provides the "margin of excellence" which sets Community Health Partners apart as the premier provider of true, community-based, comprehensive health care and in its level of commitment to the underserved. Through all its activities, the Foundation strives to give tangible expression to the Christian charitable health care mission established over 100 years ago in the greater Lorain area.
Growth
Each year the Foundation grants more than $1.2 million to support programs and services of Community Health Partners including:
- The Family Outreach Center
- New Life Hospice
- The Rehabilitation Center
- The Health Ministry Program
- Center for Body, Mind & Spirit
- The Community Cancer Center
In addition, the Foundation administers an employee scholarship program funded by generous donors who choose to express appreciation to employees in this way.
Scholarships
Community Health Partners Regional Foundation is accepting applications for the following scholarships from January 21 through April 7, 2008:
- The Paul C. Balcom Scholarship - For dependent children of full-time employees of Community Health Partners Regional Health System or a Lorain County, Ohio affiliate.
- The Francis Patterson, M.D. Memorial Scholarship - For individuals inside and outside the Community Health Partners Regional Health System with first preference given to those interested in pursuing a career in the healing arts.
- Velma Price Memorial Scholarship - For African American individuals inside and outside the Community Health Partners Regional Health System who demonstrate average or better academic achievement.
- Denis A. Radefeld, M.D. Humanitarian Award - For associates of Community Health Partners Regional Health System participating in a project providing direct care to the poor and underserved.
The deadline for all scholarships and awards is April 7, 2008.

Ways to Give
If you place Community Health Partners among your charitable interests, the Trustees of the Foundation invite your consideration of one or more of the following ways to support our programs and services.
- Annual Fund Drive
- Memorial or Tribute gifts
- Participation in the Eagle Seekers Golf Classic and other special fund raisers
- Planned and deferred gifts (bequests, trusts, etc.)
