The Community Health Partners Regional Foundation
PAUL C. BALCOM SCHOLARSHIP
This scholarship is opened to all dependent children of full time employees of the Community Health Partners Regional Health System.
- Applicant must be a legally dependent child of a full time employee of the Community Health Partners Regional Health System or a Lorain County, Ohio affiliate.
- Applicant must demonstrate average or better in academic achievement.
- Applicant must satisfy the Review Committee as to the relevance of the proposed course of study to the applicant’s personal and career goals.
- Applicants will be honored for collegiate or vocational studies.
- Grants will be awarded without regard to demonstration of financial need.
- The Review Committee will consider the potential for successful completion of the course of study to grant assistance.
- Independent application will be required for each grant review submittal
- Grant award checks will be co-made to the scholarship/grant recipient and the educational center sponsoring the educational course of study.

THE FRANCIS PATTERSON, M.D. MEMORIAL SCHOLARSHIP
This scholarship is opened to individuals inside and outside of the Community Health Partners Regional Health System.
- First preference shall be given to those students interested in pursuing career preparation in the healing arts. All other vocations and professions may be considered by the committee failing applications which qualify for first preference considerations.
- Applications will be honored for collegiate or vocational certification course work.
- Demonstrated financial need may be considered by the committee, but is not an eligibility requirement (Documentation as part of application is optional)
- This scholarship may be awarded for up to four years upon successful re-application in which students document a 2.5 GPA or better and that they meet all the certification or degree eligibility requirements of the educational institute (i.e., have not become ineligible for the degree or certificate for any reason).
- As further guidance, it shall be policy to remit the scholarship award check payable both to the educational institution and the student to ensure that funds are used for the intended purpose.

THE VELMA PRICE MEMORIAL SCHOLARSHIP
This scholarship is opened to African American individuals inside and outside of the Community Health Partners Regional Health System.
- Applicant must demonstrate average or better in academic achievement
- Applicant must satisfy the Review Committee as to the relevance of the proposed course of study to the applicant’s personal and career goals.
- Applications will be honored for collegiate or vocational studies.
- Grants will be awarded without regard to demonstration of financial need.

THE DENIS A. RADEFELD HUMANITARIAN AWARD
This award provides a Community Health Partners’ associate with up to two weeks paid leave as well as a stipend to be used toward qualified expenses to participate in a project providing direct care to the poor and underserved.
- Applicant must have at least 5 years service with the organization.
- The project must be approved as providing direct care for the poor and underserved and be consistent with the mission and values of Community Health Partners Regional Health System.
- This award is not restricted to clinical staff, however, applicants must demonstrate they will be involved in providing direct care.
- Proposed expenses to be covered by the stipend shall be limited to room, meals and travel directly related to the project to qualify. Clothing and personal equipment do not qualify.
