The Nurse Practitioner/Wound Center Coordinator provides wound care management at designated CHP Wound Center facilities. The Coordinator will manage a case load of patients at long term facilities, long term acute care hospital units and patients at the Wound Center outpatient and/or satellite clinics. Supervises case managers assigned to his/her outpatient clinics and serves as a consultant to wound care patients within the hospital. Performs wound assessment, directs and supervises wound care treatments, obtains patient health histories, performs physical examinations, identifies and differentiates between normal and abnormal findings, performs specialized procedures, collaborates with the Wound Center physicians and staff, participates in staff education.
Minimum Knowledge, Skills and Abilities
Certified Nurse Practitioner or PA-C
3-5 years of clinical experience with progressively more responsibilities
Supervisory experience preferred.
Certificate of authority; certificate to prescribe preferred
Geriatric and wound care experience a plus. |
