7-minute Health Risk Assessment

The HEARTaware evaluation will tell you if you have no risk factors, one risk factor or two or more risk factors. Here is the benefit:

No risk factors – Your gift is peace of mind.

One risk factor – You will receive regular information on heart-healthy things you can do to lessen future risk.

Two or more risk factors – You receive a free screening (if you choose) by a cardiac clinician, a referral to your physician (if you need it), and follow up by the clinician for annual screenings (if you want it) to see how you are doing. Plus you will get information on heart-healthy things you can do.

Heart Scan
If your online evaluation shows you are at risk for cardiovascular disease, a St. Mary’s Early Detection Center nurse will call you (with your permission) and offer you a more detailed screening test using a CT scanner to obtain your calcium score. Calcium scoring measures calcium buildup on artery walls and is a highly reliable predictor of heart disease. To help as many people as possible have the CT scan screening, St. Mary’s Early Detection Center offers it for a special reduced cost of $99. Click here for more information.

How do I start?
Click here to take the online risk evaluation. At the end of the evaluation, you will be asked if you want us to follow up with you. If you agree (and only if you agree), you will receive follow-up services, all FREE.

Is participation confidential?
Yes. All information is between you and St. Mary's.

What if the evaluation shows I’m at risk for heart disease?
A St. Mary's representative will contact you and set up a 15 to 30 minute screening and consultation with a cardiac nurse.

What happens at the screening?
You are given a painless blood test to check your cholesterol levels and your blood pressure is measured. The clinician will ask you about your medical history as it pertains to cardiac care.

Is there follow-up after the screening?
Yes, you will receive a confidential report. If the clinician thinks you should see a physician, you will be referred to your family physician, all within the context of your medical plan.

If I need care, is it covered by my health insurance?
All care is provided within your current medical plan.
