Lourdes offers online pre-registration options to streamline your time spent at Registration!

When you arrive for surgery, diagnostic testing, admission, or the delivery of your baby, we know you don’t want to deal with lots of paperwork. Use our online pre-registration service and experience the comfort of knowing your registration information will be processed quickly.

Just follow these steps to streamline your registration time:

• Complete and submit the pre-registration form.
• Our Registration staff will contact you regarding your insurance information. We will be happy to answer questions about your upcoming visit to Lourdes at this time.
• On your date of service/admission date, come to the Patient Registration area to sign your records. We will make a copy of your insurance cards and then promptly transport you to the appropriate department for your services.
