Fee-For-Service EAP Model - A pay-as-you-go Option
A variety of service agreements are available on a non-captitated (no annual contract) basis,. so that we can tailer an EAP model to meet your needs and resources. This FEE-for-Service arrangement services are purchased on an as-needed basis. Payment is either received prior to service delivery, or invoices are submitted for monthly payment.
This option should be thoroughly review with your Life Management Systems sales representative, as it is often more costly than eith the Standard EAP Model or the Extended EAP Model.
Fee-for Service Options include:
- Assessment and Referral Evaluation (3 sessions - flat fee)
- SAP Evaluations (3 sessions - flat fee)
- Customized Seminars
- On-Site Sevices (CISD, Management Consultation, Policy Consultation)
- Drug-free workplace implementation and training
Per Capita / Fee-for-Service Combination
A per capita (annual) fee is charged for program implementation, training, promotion and administrative services. Employee assessment, counseling, visits, and case consultation services are paid on a per session basis and invoiced monthly.
