Originally opened in 1890 by the Sisters of Mercy, Sacred Heart Village continues to offer compassionate care while incorporating the spirit of Louisville. As a partner with the University of Louisville Medical School, Sacred Heart Village has the great privilege of teaching the medical students and passing on their expertise. The wide and bright hallways provide warmth and a home-like feel, and the enclosed courtyards offer a safe walking and resting place for residents that enjoy the outdoors.

Post Acute Transitional Unit
 
Services are targeted toward the individual needing intensive nursing or therapy services after discharge from a local hospital. Services available in this unit include: 
  • Skilled nursing services for wound care, IV therapy, tube feedings and other nursing services
  • Physical, Occupational and Speech therapy services for rehabilitation and optimization of the funcation level of the individual that are goal intensive
  • Nutritionally-balanced meals with oversight provided by a Registered Dietitian
  • Assistance provided with all Activities of Daily Living (ADL's), including: bathing, dressing, incontinence care, eating, mobility and other activities as needed by the individual
  • Activities Program, Spiritual Care/Mission and Holistic Care services
Long-Term Care Unit/Skilled Nursing Unit
 
Services are targeted toward the individual needing long-term nursing services. Services available include: 
  • Nursing services for medication administration and management; insulin administration, skin care; and individualized services as needed
  • Restorative nursing services work in conjunction with therapy to provide ongoing exercise and maintenance therapies to ensure that the individual maintains their optimal level of physical ability
  • Assistance is provided with all activities of daily living including: bathing, dressing, incontinence care, eating, mobility, and other activities based on an individual's needs
  • Nutritionally-balanced meals provided with oversight of a Registered Dietitian
  • Maintenance of a clean and pleasant environment by housekeeping services
  • Activities programs to provide entertainment and build community among our residents
Additional features that enhance the everyday care provided by Sacred Hearts' professional staff include:

Apartment Living
 
Sacred Heart Village apartments provide a welcoming home for older adults to live independently in a safe, secure environment. In the tradition of the Sisters of Mercy, the apartments serve limited-income senior adults. Residents enjoy socializing with other residents in various activities offered each day. Some of the activities include:
  • Relaxation exercises
  • Readings
  • Bingo
  • Birthday Party Celebrations
  • Pot Luck Dinners
  • Speakers
  • Access to a Bookmobile
  • Daily spiritual services in our Chapel
  • Seasonal parties
Sacred Heart Village includes three locations:
 
Village I. Opened in October of 1996. The 50 one-bedroom units were build for persons 62 years of age or older, including physically challenged individuals who were living on fixed incomes. The apartments expanded by opening Village II.
 
Village II. 50 additional one-bedroom units opened in July of 2001.
 
Village III. Village III, located in the Shively area, opened in April of 2002 offering 50 additional apartment units.
 
All three apartment facilities are Section 202 and HUD subsidized for the elderly. A scale of income limits is set by HUD each year. The cost of rent is determined by total annual income minus medical expenses.
 
Some of the services available to our apartment residents include:
  • Beauty salon services available two days a week
  • Available to participate in monthly food commodity program
  • Coin-operated washers and dryers
  • Library and Bookmobile services
  • On-site Chapel 

Residential Living
 
Services available in the Residential area of Sacred Heart Village are aimed at the individual in need of a secure, safe environment with social activities available . Services in the residential area include: 
  • Activities are coordinated and planned for this group of individuals including outings, bingo, special events, birthday celebrations, and much more.
  • All meals are provided and a Certified Dietary Manager ensures nutritional balance.
  • Light housekeeping services are available to assist with maintaining a clean and pleasant environment.
  • Laundry facilities are available on-site

Adult Day Services
 
Continuing the commitment to providing excellent care for senior members of the community, DayBreak Adult Day Health Center was established in 1998 through collaborative efforts of Mercy and the Catholic Charities Aging Ministries of the Archdiocese of Louisville.
 
DayBreak provides an alternative to care in the home and/or placement in a personal care or nursing facility. DayBreak also gives respite for caregivers so their personal needs may be met. At DayBreak, participants gain the opportunity to interact with peers and engage in stimulating group and individual activities.
 
DayBreak is dedicated to: 
  • Enhancing the emotional, social, physical and spritual well-being of participants
  • Providing participants the opportunity to interact with peers and other individuals
  • Continuing the spirit and tradition of the foundresses of the Sisters of Mercy
Clientele ranges from seniors who are socially isolated and want more to do during the day, to those who are frail and need the extra supervision. We accept clients with dementia.
 
DayBreak services include:
  • Open 7:30 am - 5:30 pm, Monday through Friday
  • Medical assessment and monitoring by a licensed nurse
  • Nutritious lunch and snacks
  • One-on-one and group activities offered based on indivdiual needs and preferences
  • Transportation (wheelchair-accessible van) available for an extra charge 
  • Assistance available with personal needs such as grooming, toileting, and eating 
  • Prayer service offered daily
  • Outpatient speech, occupational, and physical and Holistic Touch Therapies available on site. 
Admission
 
Participants are asked to make a commitment to a 30-day trial period. They can choose which day(s) of the week (minimum of two days per week) they would like to attend. This trial period will help participants and caregivers determine if DayBreak is able to meet their needs. The staff is committed to making this adjustment period a pleasant and meaningful experience.
 
Funding Opportunities
 
Participants may be eligible for funding through the Medicaid Waiver Home and Community Based Waiver Program.  Additional financial information may be obtained by contacting DayBreak director, Susan Tahaney Gaither, MSSW,  at 502/895-5228 or by e-mail at sgaither@health-partners.org.
Spiritual Care
 
Spiritual care services are very important to our residents at Sacred Heart Village. Included in these services are: 
  • Daily Mass or Communion services
  • Communion for those unable to attend Mass
  • Opportunities for recitation of the Rosary
  • Ecumenical Services
  • Sacramental Services such as Annointing of the Sick and Penance
  • Weekly Growth in Faith series
  • Seasonal devotionals
Holistic Care Ministry
 
Holistic care seeks to utilize Touch Therapies to emphasize the body, mind and spiritual needs of the individual. This type of therapy can help with pain management, enhancement of physical, emotional, and spiritual well-being, and assits with moderation of fear, panic and anxiety.
