Home Health
A non-profit, hospital-based, certified agency, Mercy Home Health provides specialized care and support services to adults, children, and infants in the comfort of their own home. Skilled nursing care, a full range of therapies, wound care, and health care education are among the many services administered by our staff of nurses, medical social workers, home health aides, physical therapists, occupational therapists, and speech therapists under the direction of the patient's physician in the home.
Our patients must be under the supervision of a licensed physician and have a safe home environment that allows for effective, intermittent health care. Patients can be referred to Mercy Home Health through physicians, nurses, social agencies, hospitals, or friends and family. If the patient is hospitalized, we will work with the social worker or discharge planner to ensure the continuum of care from hospital to home. Home care visits are covered by most private health insurance, Medicare, Medicaid, workman's compensation, veteran's benefits, HMOs, and private funds.
Services
Mercy Home Health professionals help patients and their families safely provide the care that is needed, where they are most comfortable -- at home. Educating the patient and/or the family about the patient's condition and required care is an important part of this process. Mercy Home Health's broad range of services includes:
- skilled nursing care
- physical therapy
- occupational therapy
- speech therapy
- chemotherapy
- intravenous therapy
- parenteral and enteral nutrition
- diabetic management
- colostomy care
- wound care
- cardiac care (EKG/Holter Monitors)
- infectious disease
- laboratory
- medical social work
- home health aides
- durable medical equipment
Our empathetic and highly professional nurses, therapists, social workers and home health aides are trained to consider not only the patient, but also the family. By assessing the family's coping mechanisms and teaching members how best to care for the patient, our staff enhances the patient's support system, while easing the stress that an acute illness or injury can bring to a home. Mercy Home Health staff works closely with each patient's physician, notifying the physician by phone of any change in the patient's condition, and sending written reports on a regular basis.
The Mercy Difference
Mercy Home Health is an important link in Mercy Health Partners' Continuum of Care, as it responds to the physical and emotional needs of both the patient and the patient's family. Nowhere is Mercy's commitment to providing whatever care is needed, whenever it is needed, more apparent.
