One-on-one education for individuals or families. This service is provided on an outpatient basis for two to three visits total.

Social Services - St. Vincent Mercy Medical Center
The department assists patients and families to make the transition from pre-admission to post hospital services including coordinating the efforts of service providers to meet the continuing needs of patients and families once they are discharged from the hospital. The Social Services Department also provides counseling and support.

The Transitional Care Unit (TCU) is located on the third floor of Mercy Hospital of Tiffin. This unique unit is used for rehabilitation before your return home. It is located within the hospital so that your primary care physician can see you when needed.
