The mission of Community Outreach Program is to improve the health of the community by assessing health needs and working within the community to address those needs. Community Health Education
Professional educators provide community programs and screenings to enhance health and wellness. Programs include Women in Touch, Safe Sitter, One Step at a Time Walk Fitness, and community health events for men and women. Screenings include cholesterol, blood pressure, osteoporosis, body fat and blood sugar. For more information, call 330-480-3070.
Diabetes Outreach and Education
When you are diagnosed with diabetes or pre-diabetes, Humility of Mary Health Partners Diabetes Education program can help you create a healthy lifestyle in a variety of ways: day and evening education classes focusing on diet/meal planning, insulin instruction and glucose monitoring; a Pathways to Success program to assist women with gestational diabetes; a diabetes support group; and an outpatient insulin pump program.
Diabetes Education program is a superior program, having received the Education Recognition Certificate by the American Diabetes Association for meeting national standards established for diabetes self-management programs. For more information, call 330-480-2676.
Prescription Drug Assistance Program (PDAP)
The Prescription Drug Assistance program (PDAP) is a free service available to assist senior citizens in Mahoning and Trumbull counties get medications used on a long-term basis from drug companies that provide free or low cost drugs to those who cannot afford them. Program representatives aid participants in determining if they qualify and completing the necessary forms.
The Trumbull County PDAP is administered by Trumbull County SCOPE. For more information, call 330-399-8846.

Neighborhood Health Watch
Neighborhood Health Watch is a free service to the minority community with a focus on meeting the needs of the poor and underserved. A trained health care provider makes regular visits to locations in Mahoning and Trumbull counties to provide health screenings, health education and referrals to health agencies. Resource Mothers and Women, Infants and Children (WIC)
The Resource Mothers program and WIC provide assistance for mothers and their young children. The main clinic for Trumbull County's WIC (Women, Infants and Children) and Resource Mothers programs are now located inside the Gibson Building, 258 E. Market St., Warren. WIC also has an office at 979 Tibbetts-Wick Rd., Girard. For more information, call 330-394-0990 (Warren) or 330-539-1010 (Girard).

Supportive Care
Supportive Care is a consultative program that provides comfort and compassionate care for people diagnosed with a chronic or life-threatening illness. Designed to be a support system that helps patients and families every step of the way, Supportive Care puts people in charge of their health care and does not interfere with their current medical care.
The Supportive Care team helps patients live as actively as possible, assists in pain/symptom control, provides education and access to community resources, and offers ongoing support throughout the course of the illness. Supportive Care referrals can be made by any member of the healthcare team, the patient or family members. For more information, call the Supportive Care program coordinator at 330-480-2199.
