Home Care Rehabilitation
As the tri-county's largest home health care agency, HM Home Care provides care to individuals who are in need of rehabilitation, but are unable to safely and effectively leave their home. We provide service to individuals with any condition limiting their mobility or restricting their ability to leave their home environment. For individuals to be eligible for this service, the patient must be referred by a licensed physician and may need to meet certain Medicare and insurance guidelines for homebound status.
